Fill out the short 5 minute survey below and you’ll be entered into a monthly drawing for a 6 ball pack of Chromax! We will pick one entry each month and send him or her some balls on us! Easy!

The rules:

• Fill out the survey. Answer the questions completely and provide your contact information at the end of the survey (information will not be sold or spammed!).
• Limit one entry per address. If you fill out more than one survey you’ll be automatically disqualified. Be fair!
• Entries will not expire. If you don’t win this month, we’ll keep your entry until the end of this survey period so no need to fill out the same survey over and over!
• Contest is open to anyone in the 50 States, 18 or older. Sorry, we don’t make this rule (we hate excluding anyone), so be mad at our lawyers and contest laws if you have to be mad.
• Winner will be chosen randomly every month. The winner will receive one (1) pack of Chromax Golf balls (6 assorted colors). Approximate retail value is $19.95. Awesome!
• If you are chosen, you will be notified by email. Winners must respond within seven (7) days of email. If you do not respond within that period, another winner will be chosen.
• No purchase necessary. Void where prohibited. Odds of winning depend on the number of eligible entries received.
